Estimate Medicare monthly and annual cost by age, plan choice, income for IRMAA and location with latest CMS Part A, Part B, Part D and Medigap data for USA.
Why Medicare Costs Feel Different In Texas Florida And California Homes
A 70 year old in Austin Texas ZIP 78701 with income 95,000 dollars single filing, premium free Part A, choosing Original Medicare plus Medigap Plan G at 150 dollars and Part D at 42 dollars, pays a different monthly total than same person in Los Angeles California ZIP 90001 and Miami Florida ZIP 33101. Federal base is identical across states. Part A premium free is 0 dollars when 40 quarters are met, Part B standard premium latest is 185 dollars monthly, Part B deductible latest 257 dollars, Part A inpatient deductible latest 1,676 dollars per benefit period. That base does not change by state.
What changes is plan pricing and availability. Texas average Medigap Plan G around 140 to 180 dollars at age 70, California around 170 to 230 dollars due to birthday rule and community rating variations, Florida around 160 to 220 dollars. Part D premiums also vary by ZIP. Austin may see Part D plans from 15 dollars to 85 dollars, Los Angeles 20 dollars to 95 dollars, Miami 18 dollars to 90 dollars. Medicare Advantage premiums show even wider spread. Texas often has 0 dollar premium Advantage plans with drug coverage, California similar but with more 0 dollar plans in metro counties, Florida also heavily 0 dollar premium but with higher out of pocket maximums.
IRMAA makes income the biggest driver. Take same 70 year old with income 95,000 dollars single. Threshold for first IRMAA tier for single is 106,000 dollars, so income 95,000 stays under threshold, IRMAA 0 dollars. If income were 140,000 dollars single, Part B IRMAA adds about 74 dollars monthly and Part D IRMAA adds about 13.70 dollars monthly. That lifts monthly total from about 377 dollars to about 465 dollars for Original plus Medigap plus Part D. For joint filing thresholds double, first threshold 212,000 dollars, second 266,000 dollars. A couple in Dallas Texas ZIP 75001 filing joint with combined income 250,000 dollars would be in first IRMAA tier, adding same 74 dollars plus 13.70 dollars to each person who has Part B and Part D.
California has unique Medigap rules with annual birthday window allowing switch without underwriting, which can lower long term cost if health changes. Texas uses attained age rating for many carriers, so premium rises with age. Florida uses issue age rating for some carriers but attained age for others, making initial quote comparison essential. These state patterns do not affect CMS base but affect the plan premium you enter into this calculator.
What Drives Your Medicare Monthly Total Inside Real Formula
First driver is Part A premium status. Most people get premium free Part A after 40 quarters of Medicare covered work. If 30 to 39 quarters, Part A premium latest about 285 dollars monthly. If fewer than 30 quarters, latest about 518 dollars monthly. Most 65 plus have premium free. If you are still working and have employer coverage, you may delay Part B but Part A is usually still premium free.
Second driver is Part B base premium and deductible. Standard Part B premium latest 185 dollars monthly for most beneficiaries. Part B deductible latest 257 dollars per cycle. After deductible, Part B usually covers 80 percent, you cover 20 percent unless Medigap or Advantage changes cost sharing. This 185 dollars is the same for everyone except IRMAA addition and late enrollment penalty if you delayed without creditable coverage.
Third driver is IRMAA. IRMAA tiers for single: under 106,000 dollars no addition, 106,000 to 133,000 dollars Part B plus about 74 dollars, 133,000 to 167,000 dollars plus about 185 dollars, 167,000 to 200,000 dollars plus about 278 dollars, 200,000 to 500,000 dollars plus about 371 dollars, above 500,000 dollars plus about 406 dollars. Part D IRMAA in same brackets adds about 13.70, 35.30, 57.00, 78.60, 85.80 dollars. For joint, thresholds are double except top bracket at 750,000 dollars joint. For married separate, thresholds are much lower, often starting at 106,000 dollars with higher surcharge quickly.
Fourth driver is Part D premium. National average base around 36 dollars but plan bids range from under 10 dollars to over 100 dollars. Your actual Part D premium plus IRMAA for Part D equals monthly drug coverage cost. Low income subsidy can reduce Part D to 0 dollars for qualifying incomes.
Fifth driver is plan choice. Original Medicare only has no extra premium beyond Part A and Part B plus Part D, but leaves 20 percent coinsurance uncapped. Medigap Plan G latest average for 70 year old about 150 dollars monthly in Texas, 185 dollars in California, 175 dollars in Florida, but fills most gaps. Plan N cheaper by 20 to 40 dollars monthly but has copays. Medicare Advantage monthly premium latest average about 25 dollars nationally, many 0 dollar plans, but out of pocket maximum up to 8,850 dollars for in network and higher combined. Your expected doctor visits and prescriptions tilt which total is lower.
How To Use Calculator Without Mistake
First, select age and filing status. Age affects Medigap quote you should enter, filing status affects IRMAA tier thresholds. Single threshold starts at 106,000 dollars, joint at 212,000 dollars.
Second, enter annual income used for IRMAA. Use modified adjusted gross income from two cycles prior. Include tax exempt interest. If your income recently dropped due to retirement, you can request reconsideration with Social Security using Form SSA-44 for life changing event.

Three Household Examples
Austin Texas 78701 Age 70 Single Income 95,000 Premium Free Part A Original plus Medigap: Part A 0, Part B 185, IRMAA 0, Part D 42, Medigap 150, monthly 377, annual with 257 plus 1200 OOP about 5,981. No IRMAA.
Los Angeles California 90001 Age 70 Single Income 140,000 Premium Free Original plus Medigap: Part A 0, Part B 185 plus 74 IRMAA equals 259, Part D 42 plus 13.70 equals 55.70, Medigap 185, monthly about 499.70, annual about 7,454. IRMAA tier 1.
Miami Florida 33101 Age 75 Joint Income 250,000 Premium Free Advantage: Part A 0, Part B 185 plus 74 IRMAA, Part D 45 plus 13.70, Advantage 25, monthly about 342.70, annual with 1,457 OOP about 5,569. First joint tier.
Mistakes That Break Medicare Math
- Forgetting IRMAA uses 2 prior cycles: Current income may be lower after retirement, but IRMAA uses income from two cycles ago. Request reconsideration if income dropped due to work stoppage.
- Mixing Medigap and Advantage together: You cannot use both at same time. Medigap works only with Original Medicare, not with Advantage. Calculator uses one or the other based on coverage choice.
- Using Part B premium without IRMAA for high income: Income above 106,000 single or 212,000 joint adds surcharge. Ignoring understates monthly by 74 to over 400 dollars.
- Ignoring Part A premium if not premium free: If fewer than 40 quarters, Part A adds 285 to 518 dollars monthly, often missed for immigrants or late workforce entry.
- Comparing only monthly premium not annual exposure: 0 dollar Advantage premium can still have 5,000 dollar out of pocket maximum. Original plus Medigap higher monthly but lower point of service cost. Always compare annual total.
How To Budget After Estimate
Use monthly total to compare with Social Security benefit. If Social Security is 2,200 dollars monthly and Medicare total is 377 dollars, remaining for other living costs is 1,823 dollars. If IRMAA pushes total to 500 dollars, remaining drops to 1,700 dollars. Set aside Part B deductible at start of cycle and Part A deductible if hospital risk is higher. For Part D, check formulary tiers and pharmacy network, as actual drug cost can exceed premium. If income near IRMAA threshold, consider Roth conversion timing and capital gains timing as they affect modified adjusted gross income. Texas residents with Health Savings Account history should note HSA contributions must stop 6 months before Part A start if retroactive.
